The protagonist is —a former "Hollow Courier" who, after a delivery gone wrong in the subterranean Briar Womb, becomes infected with a sentient, carnivorous parasite. The core gameplay loop revolves around balancing Puck’s humanity against the parasite’s hunger. You can either feed the parasite to gain horrific powers (tentacle lashes, acid spit) or suppress it with rare Antiserum to keep Puck’s dialogue and ending humane.
